If you are new a patient to our office, please print and fill out the forms and bring them with you. Also remember to when you come for your appointment. Please bring your insurance card/s and referrals if applicable. This will save you waiting time.

If you would like to use your insurance(s), please print and fill out the form. Insurance is not a guaranteed payment! Please contact your insurance company for coverage information before your appointment. Most of the medical insurances have high deductibles and therefore your office visits will not be covered. Your routine eye exams will be covered or partially covered by VSP and EyeMed. Some medical insurances will also cover routine eye exams. Myopia progression control and orthoK will not be covered by any insurances!
